Summary of responsibilities:


The Vocational Specialist works on the Assertive Community Treatment Team (ACTT), an interdisciplinary team that operates with a client care delivery model facilitating community living, psychosocial rehabilitation, and recovery for people with serious and persistent mental illness.

The Vocational Specialist provides client-centered, recovery-oriented care and assists clients with job readiness, job coaching, and mentoring, as well as supporting coordination with Ontario Disabilities support employment program, employment agencies, volunteer services and employers.

The Vocational Specialist conducts client assessment of interests, aptitude and motivation, as well as financial, social, medical and vocational histories to formulate and support follow through on client vocational treatment plans.


Requirements of the job:


Requirements:

  • 2year College diploma in a health/social science related field
  • Minimum 2 years' experience providing vocational rehabilitation services to adults with serious and persistent mental illness.
  • Vocation studies certificate or acceptable combination of education, training and experience in vocational rehabilitation.
  • Sound knowledge of vocational rehabilitation theory and practice, including various treatment modalities and related techniques.
  • Strong assessment, interviewing and therapeutic skills.
  • Current certification in Non-Violent Crisis Intervention and CPR.
  • Ability to be organized and autonomous, and have good teamwork capabilities.
  • Excellent interpersonal and communication skills.
  • Computer literacy, and knowledge in using CRMS and CERNER.
  • Valid driver's license in good standing and reliable access to a vehicle.
  • Demonstrated regular, punctual attendance
  • Demonstrated commitment to patient safety
  • Willing to work with a variety of community partners
  • Functional proficiency in English/and French, oral and written

Assets:

  • Indepth knowledge of the Addiction, Mental Health and Social Services available in the community and referral pathways
  • ASSIST training and Specialized Suicide Intervention Training (Adult )
