Position Summary:

The Back to Home (BTH) Residential Counsellor 2 (RC2), reporting to the BTH Supervisor, works as a member of a multi
- disciplinary team and is responsible for providing residential supports and services to all clients at the BTH residence ie. support with ADLs, group design and implementation, individual support to achieve goals, excursions etc.


Schedule Summary:

Two 12-hour weekend shifts every other weekend


Program Summary:


The objective of the HPE-BTH program is to provide client-centered, community-driven solutions to reduce Alternate Levels of Care ("ALC") waitlist pressures.

This will be through providing eligible individuals with MHA ("Clients") on ALC waiting lists
(within the hospital and long-term care settings) with supportive housing in the form of:

  • Rent supplements to make housing affordable;
  • Mental Health and Addictions supports;
- home and community services; and/or
- assisted living supports.

  • Under this program, clients must receive both support services and housing assistance at the time of entering the program. However, as individuals' needs change, supports may be adjusted as appropriate.

The BTH program will include:

units, the majority of which will be for single individuals but there will be some identified for couples, no children will be residents of the homes.


  • Onsite staffing 24/7. Staff are "attached" to the building/program and will include residential staff, nurses, case managers and addictions counsellors and PSWs; and
  • Specialty and need specific services provided onsite by nonresidential staff.
  • Clients will work together with the Residential staff, Case Managers, Addictions Counsellors and other wrap around supports to develop collaborative care plans semiannually.

Minimum Qualifications & Experience:


  • College Diploma in a related discipline
  • 6 months to one year relevant experience providing services to individuals who live with a mental illness, a dual diagnosis, and/or concurrent disorder, preferably in a community setting
  • Current First Aid and Cardio Pulmonary Resuscitation (FA/CPR) Certification an asset
  • Current Non Violent Crisis Intervention (NVCI) an asset
  • Applied Suicide Intervention Skills Training (ASIST) an asset
  • Demonstrated crisis intervention skills/experience
  • Experience with communication, interpersonal and prioritization skills
  • Experience with crisis intervention, problemsolving and conflict resolution skills
  • Demonstrated ability to develop therapeutic rapport with individuals living with complex needs
  • Must be able to work independently and interdependently in collaboration with other multidisciplinary team members
  • Must possess a valid Ontario Driver's License, be able to be insured on agency owned vehicles, have regular access to a reliable vehicle and provide proof of adequate vehicle insurance
  • Basic proficiency in computer skills; MS Office preferred
  • Required to provide a satisfactory criminal reference check (CRC) with vulnerable sector check prior to hire
