What can I expect to do in this role?: As the Case Management Coordinator, you will:

  • Work collaboratively with caregivers, community professionals and our clinical team
  • Coordinate the client services processes and completing assessment tools
  • Chair and organize case meetings with multidisciplinary teams
  • Provide resources to support client and caregiver needs
  • Ensure treatment recommendations are communicated to caregivers and community agencies

How do I qualify?:


Mandatory:

Successful completion of:
- a Social Services Worker, Child Care Worker or Developmental Services Worker program or
- equivalent combination of experience and education in the treatment of children with mental health and/or developmental needs


Technical Knowledge and Experience:


  • You have an understanding of treatment of children with mental health or developmental needs
  • You know psychological theories, child behaviour assessments, and social work systems theory
  • You have knowledge of assessment methods, functions, and the relevance of clinical assessments
  • You have an understanding of social services in Ontario, specifically related to children services and can acquire knowledge of relevant legislation

Communication (oral and written), Interpersonal, and Presentation Skills:

  • You can prepare reports accurately and concisely
  • You can work effectively and collaborate with a variety of stakeholders such as professionals, staff, the public, and community representatives
  • You are able to ensure recommendations from treatments are documented and communicated to client's caregivers and community agencies

Reasoning, Analytical, Assessment Skills:


  • You can gather information and discuss treatment needs
  • You can conduct research and evaluation

Other Essential Skills:


  • You have organizational skills to coordinate and manage information and data from a variety of sources
  • You can plan and prioritize workload and work within timelines
  • You are proficient with computer software such as MS Word, Excel, and Outlook
  • Ability to travel within the service area
