Program Coordinator, Community Support Services

Summary

In support of the mission, vision, values and policies & procedures of Lambton Elderly Outreach, the Program Coordinator of Community Support Services will facilitate client intakes, schedule client's appointments, coordinate transportation, and complete monthly program reconciliation.

They will contribute to the ongoing development of the agency programs and will work as part of a team to ensure the needs of our clients are being met through quality Community Support Services.

Primary Duties

  • Receive instructions and guidance from the Manager of Community Support Services
  • Provide outstanding customer service to clients while scheduling services
  • Complete intake assessments
  • Operating a multiline phone
  • Maintaining records and general administrative duties
  • Work cooperatively with clients, visitors, staff, and volunteers while maintaining confidentiality
  • Other duties as required

  • The ability to work within established timeframes
  • Must demonstrate a desire and ability to work effectively within a team environment
  • Good organizational skills
  • Ability to multitask in a high paced work environment

Key Responsibilities:

  • To know the mission, vision, and value statement of the agency
  • To seek guidance from the Manager, Community Support Services whenever in doubt
  • To respect and maintain the confidentiality of the individuals, and/or families being serviced, as well as the organization
  • To notify the office of any illness, emergencies, or delays
  • To follow the LEO Code of Ethics, Policies and Procedures
  • To review performance with the program supervisor after probation, and yearly thereafter
  • To understand and follow the Client Bill of Rights

Requirements:


  • Completion of a satisfactory Criminal Reference Check and Vulnerable Sector Screen
  • Dependable, courteous, and punctual
  • Experience with scheduling considered an asset
  • Ability to independently problem solve
  • Advanced oral and written communication
  • Ability to remain open to other's ideas and perceptions
