SUMMARY OF POSITION
As the Housing Support Coordinator, your primary responsibility will include providing support coordination, individualized planning, supportive counselling, crisis support and overall assistance to reach recovery goals of individuals who experience mental health and substance use issues.
HOURS OF WORK
The Coordinator will work thirty-five (35) hours per week, with the hours being flexible to the needs of clients.
This position includes staff on-call responsibilities on a rotational basis.
Salary Range:
$ $33.24 per hour (Grade 5) (Based on education and experience)
ESSENTIAL QUALIFICATIONS
- Postsecondary degree or diploma in human services/human sciences, or related field.
- At least three (3) years of experience in a community mental health setting working with adults recovering from serious mental illness.
- Experience in developing and implementing program guidelines.
- Demonstrated ability to work as a highly effective communicator with strong interpersonal and teamwork skills.
- Experience working in a residential setting, and/or working with individuals who have dual diagnosis, concurrent disorders, and acquired brain injury.
- Knowledge of mental health diagnoses, substance abuse, harm reduction, and recovery focused approaches is required.
- Strong assessment skills and demonstrated ability to assess complex situations and make appropriate decisions independently
- Sound knowledge of community resources.
- Proficient in the use of Word, Excel, electronic record keeping and data collection systems.
- Certification in First Aid/CPR, Non-Violent Crisis Intervention, and suicide risk assessment preferred.
- Must have a valid Ontario driver's license, relevant insurance and use of a reliable vehicle.
- Able to successfully complete and provide a current Police Vulnerable Sector Check.
