We currently have a
Regular Full-Time opportunity for a
Social Worker in the
Reactivation Care Centre department at our
Humber Church campus.
Hours of Work:
Days, 8hrs; As per schedule
Summary of Duties
Provision of Social Work services to patients and families at an advanced level of practice and within an interprofessional model
This includes:
supporting patients and families to meet their emotional and practical needs from admission until end of life, completion of psychosocial assessments, counselling, group work, locating and arranging resources, consultation and collaboration with individuals and teams, teaching/education, research, project work, administration, and professional development.
Qualifications/Skills
- Masters Degree in Social Work
- Member in good standing of the Ontario College of SW
- Previous experience working in a health care setting (preferably 2 years); geriatric / elder care and end of life experience preferred
- Cognitive support experience required
- Excellent assessment, interviewing and problem solving skills
- Excellent written and oral communication, mediation, time management and organizational skills
- Strong team building skills
- Knowledge of the health care system and community resources
- Superior ability to manage multiple tasks and set priorities in a fastpaced environment
- Commitment to person centred care
- Commitment to student education
