The Lead, Provincial Clinical Initiatives - Cardiac Ambulatory, Community & Integrated Care reports to the Senior Director, Quality, Research and Integrated Care at Cardiac Services BC (SSBC) and is responsible for leading and facilitating the development of practice and policy to drive improvements in patient care across the continuum.

This role lead and/or assist with identified projects, proposes new initiatives and contributes to all aspects of CSBC's strategic plans, including the development, integration, implementation and evaluation of clinical initiatives that support the provincial cardiovascular system of care.

The Lead is active in the development and implementation of best practices and policies to optimize healthcare system performance and clinical care provision at the provincial level.

Provides ongoing leadership and advice to regional health authority partners, clinicians, and other stakeholders by participating in the development and implementation of provincial clinical care policy and standards.

Contributes to the development and monitoring of the program strategic plan.

In collaboration with the Senior Director, provides leadership within the clinical programs and provides work direction to manages of assigned projects through all phases.

The Lead represents CSBC on local, provincial and national committees and liaises with partner organizations as required.

Ensures strong inter-professional collaboration through the development of structures, clinical support processes and mechanisms that support positive patient outcomes throughout the province.

The Lead drives change by engaging organizational leaders in appropriate discussions to communicate shared beliefs, attitudes and intention to build knowledge of successful clinical policies and initiatives.


What you'll do

  • Plans and supports implementation of best practices, supporting quality of care delivery, prioritizing provincial clinical carerelated activities and serving as a resource for best practice. Oversees provincial projects and initiatives undertaken by the program as an expert advisor and liaison for Health Authority partners across the province, covering the continuum of care for specialty populations.
  • Leads initiatives related to CSBC priorities with a focus on supporting prioritized improvements as per the program strategic plan such as device remote monitoring, highrisk heart failure. Creates innovative solutions to complex provincial clinical care delivery issues.
  • Establishes and maintains strong relationships with key provincial stakeholders to determine needs, priorities and opportunities for provincial/interregional collaboration Leads provincial knowledgetranslation related activities including implementation, evaluation and monitoring of best practices to support delivery of quality care across the province
  • Engages in consultation with professional disciplines at all levels of the organization and beyond the Health Authority to ensure that planned practices are designed to support the maintenance or enhancement of clinical care delivery. Ensures strong interprofessional collaboration through the development of structures, clinical support processes and mechanisms that support positive patient outcomes throughout the province.
  • Utilizes and models effective change management strategies to achieve systems level transformation. Provides leadership and management in the communication and availability of clinical policies and initiatives that support the provincial system.
  • Leads the development of a comprehensive ambulatory strategy, integrating all existing specialty services into a common governance structure and defining appropriate scope for specialty service and care models across the age continuum.
  • Serves as a resource for Health Authority Cardiac Leads and acts as a liaison with stakeholders across the province such as Primary/Community Care, and Chronic Disease Management. In collaboration with the Senior Director, implements action plans, tools and key communication systems for engaging stakeholders about cardiac best practices. Establishes input/feedback mechanisms for stakeholders, evaluates and makes recommendations on practice and systems.
  • Participates in the development of program related care systems at the community, regional and provincial levels. Assesses and evaluates the need for change relevant to clinical services and systems. Facilitates discussion with stakeholders to identify processes that work well and to identify opportunities for improvement.
