Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto.

PCH currently operates Camilla Care, a 236-bed long-term care (LTC) home and will soon be operating two new state-of the-art LTC homes and community hub in West Mississauga.

To be completed in 2023, the new homes will have a combined 632-beds and be part of PCH's larger strategy that will introduce innovative and inclusive programs and services and new models of care delivery.

Position Summary:

PCH is seeking a Social Worker, Behavioural Support Unit (BSU) to support the Partners Community Health team.

Wellbrook Place's Behavioural Support Unit is a transitional home area that provides specialized care for people with responsive behaviours associated with a dementia and older Adults with Mental Health Disorders.

Reporting to the Director of Care, the Social Worker collaborates with the inter-professional team to assess and develop a plan of care that meets resident psychosocial and social needs promoting the resident overall wellbeing and safety.


Key Responsibilities:


  • Assume responsibility for provision of Quality Resident Care on the resident home area (RHA) as per Camilla's policies and accountability framework
  • Support residents/families by communicating with them and involving them in the provision of their own care when possible
  • Participate in all aspects of personal resident care to ensure the physical, psychological, social, and spiritual needs of each resident/family are met under the direction of the Registered Staff and/or Director of Resident Care
  • Utilize therapeutic communication by establishing a working relationship with resident/families and coworkers for the purpose of meeting the resident's needs, ensuring respect, dignity, individuality of each resident and staff member
  • Deliver effective and efficient residentfocused care that promotes resident/family choices and acknowledges the resident's strength, limitations as well as their need for safety
  • Participate with other team members in formulating, delivering, and reporting the daily resident care needs and preferences as per the individual's plan of care
  • Participate in continuous quality improvement of the RHA and associated care services
  • Participate in mandatory education/training inservices for skill development and updates on new and/or relevant information on best practices

Qualifications:


  • Current Personal Support Worker Certificate from a recognized institute
  • Completion of sixhundred and forty (640) hours in PSW curriculum
  • Minimum 1 year experience in a Long-Term Care setting an asset
  • Knowledge of Ministry of Health and Long-Term Care (MOHLTC) standards
  • Ability to work well with others in a team approach and adapt to changing situations
  • Ability to work effectively in a multidisciplinary stressful environment
  • Excellent communication skills (written and oral), interpersonal skills and computer literacy
  • Ability to demonstrate an antioppression approach to practice
