EHN Outpatient Leadership Team Position Summary: The Aftercare Manager is responsible for the administrative and clinical leadership of the EHN Aftercare program. This position will be involved in the coordination, maintenance and scale-up of an evidence-based aftercare program for clients who have completed an inpatient or Intensive Outpatient Program at EHN Canada.


Job Function and Duties:


  • Work directly with clients and referents (if applicable in conjunction with inpatient staff to coordinate the implementation of aftercare programming
  • Responsible for implementation, administration and evaluation of the aftercare program
  • Support Operations Director in planning and implementing outcome measurement in the aftercare program
  • Collaborate with Clinical Director and Program Managers to develop aftercare clinical content based on evidenced informed best practice
  • Providing clinical supervision and support to aftercare clinicians to support the optimal delivery of evidencebased clinical content
  • Collaborates with the Operations Director on the development of the aftercare group schedule that meets staff and client needs
  • Addressing clinical issues with clients and referents as they arise and field complaints, engaging in service recovery
  • Conducting audits and surveys for quality assessment in clinical documentation and processes
  • Creating and disseminating aftercare policies, protocol and process flows
  • Audit aftercare groups to ensure quality care
  • Work collaboratively with EHN inpatient and outpatient facilities
  • Work with national director, operations director and inpatient facilities to problem solve workflow and client issues
  • Implementing On-Boarding Processes for Intensive Outpatient Programs
  • Overseeing the On-Boarding Team to ensure quality and efficiency
  • Overseeing both the Socialization and Stabilization and Relapse Prevention Groups
  • Overseeing Monitoring Program (Soberlink and SureHire)
  • Oversee Aftercare Dashboard to ensure it is up to date
  • Maintaining relationships with Aftercare referents and coordinating Aftercare Reporting

Other duties as assigned Qualifications:


  • Must have a degree in social work, occupational therapy, psychology or similar, and must be registered with a regulatory body, masters degree preferred
  • 5 years experience in leadership and leading clinical teams, supporting professional development, performance and clinical supervision
