Eden House Care Facility, located in Guelph, Ontario is a 58-bed long-term care home dedicated to positively enriching the lives of our residents in partnership with UniversalCare, our family members, valued employees and community partners.

Our team is currently hiring for a permanent part-time RAI Coordinator.

Reporting to the Director of Care, the RAI Coordinator position is a critical role with overall accountability for the planning, directing and coordinating of activities that support the efficiencies of the nursing care team and the safe and effective delivery of care to our residents.


As part of our team, your role as a RAI Coordinator contributes to our resident experiences in the following ways:

  • Coordinating the completion of the RAI MDS assessments for all residents within established timelines.
  • Generating RAI MDS 2.0 reports and facilitating their use in clinical and operational decision making.
  • Oversees the development and coordination of the data submission process.
  • Supervising and conducting orientation of new RN's, RPN's, PSW's and other staff in relation to MDS RAI 2.0 documentation requirements.
  • Ensures MDS assessments, resident assessment protocols and plan of care reviews are completed in a timely manner and in accordance with CIHI guidelines and MOHLTC legislation.
  • Assists RAI users to correctly code MDS assessments and provides support for validation of coding.
  • Ensures a resident assessment schedule is developed, completed, current and accessible to all RAI users.
  • Integrates MDS assessments with existing clinical tools and provides leadership in streamlining associated processes.
  • Regularly communicates with the leadership team and all RAI users across all shifts.
  • Monitors and provides support to all RAI users across all shifts.
  • Assisting the Director of Care in overseeing that admission procedures are completed.
  • Works with RAI users to develop processes that ensure data accuracy, efficient data collection, assessment completion and data utilization.
  • Coordinating a schedule, then completing and analyzing monthly/quarterly audits and indicators; identifying and creating corrective actions with the Director of Care, as required.
  • Participates as a member for assigned committees.
  • Working in accordance with the organization's Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
  • Performs other duties as assigned.

Qualifications:


  • Must possess current registration with the College of Nurses of Ontario as a Registered Practical Nurse.
  • Sound knowledge and experience with implementation of educational strategies and adult facilitation.
  • Must possess three (3) years related work experience, with exposure to clinical, project leadership or managerial experience.
  • Proficiency with the classification process and funding indicators.
  • Strong leadership abilities and highly proficient time management and organizing skills.
  • Must have a minimum of two (2) years longterm care experience.
  • Sound knowledge of the Long-Term Care Homes Act 2007, Ontario Regulations 79/10 and MOHLTC program.
  • Must posses excellent written and oral communication skills, problem solving abilities and be detail oriented.
  • Ability to work with interdisciplinary team members and other internal stakeholders.
  • Proficiency with computers and experience with clinical information systems.
  • All applicants must successfully pass the prescribed Vulnerable Sector check.
  • All applicants must provide proof of being fully vaccinated, except due to legitimate established exceptions.
