About the Position:


  • The Case Manager, Adult Protective Services Worker (APSW) is accountable for establishing a voluntary working relationship with adults 18 years of age or older who have a developmental disability and are living on their own or are transitioning to live on their own in the community. The CM, APSW assists each client in strengthening their capacity to manage and acquire the skills necessary for housing stability, daily living and their awareness of and access to generic communitybased resources and governmentfunded services and supports (e.g. the Ontario Disability Support Program, Passport Program, Legal Aid, Ontario Works etc.). The CM, APSW supports the individual to develop a network of supports that will foster greater personal independence and social inclusion.
  • People typically rely on a CM, APSW for support related to:
  • Acquiring and maintaining housing
  • Accessing generic community services (e.g., legal, housing, recreation) and governmentfunded programs
  • Health, safety and security
  • Financial management, budgeting and planning
  • Parenting and family issues
  • Acquiring and maintaining social and life skills
  • Social and recreational support; and
  • Attending medical, legal, financial and other appointments
  • The Case Manager, Adult Protective Service Worker is expected to coordinate and provide support services in accordance with Agency and Ministry of Community and Social Services guidelines _2012 Policy Guidelines for the Adult Protective Service Worker Program_.

Primary Responsibilities:


  • Reviewing the referral information of applicants to the program
  • Coordinating and participating in screening interviews with applicants and their respective referral sources
  • Determining eligibility for the program and confirming acceptance of new clients into the program
  • Developing a collaborative, supportive and therapeutic relationship with each designated client for whom he/she/they are assigned primary responsibility
  • Completing a comprehensive assessment of the nature of each client's unique goals and needs through collaborative engagement with each client and their respective support networks
  • Providing guidance and assistance to each client in relation to the development of both a personalized Client Safety Plan and a Recovery/Goal Plan
  • Supports with problemsolving, life skills (such as personal budgeting, use of transportation), general education and awarenessbuilding on abuse prevention, helping resolving landlord/tenant issues
  • Providing direct practical support, coaching and counsel to clients in their pursuit of identified goals and needs.

Some examples of these supports include:

  • Assistance in acquiring and maintaining safe/affordable housing
  • Assistance in researching and/or identifying options for supplementary community services and supports as required/requested
  • Assistance in accessing food, income, ID, clothing and other essential services/supplies in order to support each client's ability to live successfully in the community
  • Assistance in developing personal selfmanagement strategies
  • Support in the development and maintenance of Activities of Daily Living (ADLs), including but not limited to personal budgeting, use of public transportation, housing maintenance/tenancy responsibilities, food and nutrition, parenting (if/as applicable), sexual health, safety and relationships
  • Assistance in developing a healthy support network
  • Liaising with each client's support network members as appropriate and where client consent has been obtained
  • Assisting clients with emergency and/or crisis needs as required/appropriate
  • Assistance in accessing primary health/dental care
  • Assistance in accessing psychiatric care (if/when needed)
  • Assistance in developing personal skills in the areas of budgeting, medication management, etc.
