Position Description:


Temporary Assignment - approximately 8 months
***: The Social Work Discharge & Transition Specialist takes a leadership and consultant role to support discharge planning processes and patient flow for patients of St. Joseph's Healthcare Hamilton (SJHH).


The role includes:

working with internal stakeholders and community partners to ensure patients' optimal transitioning to home or next level of care, evaluating outcomes to ensure quality and continuity of care, integrating research and best practices, developing processes to support and ensure accurate reporting of data (specifically Alternate Level of Care), contributing to the development of new programs, services and programs as required, and creating, utilizing and disseminating knowledge.


Some key responsibilities and accountabilities include, but are not limited to: consultation for the safe discharge of complex cases; consultation to clinical teams to ensure effectiveness and efficiency in discharge planning; education, research and quality improvement; participation in development of policies and procedures, clinical pathways, and patient information related to discharge planning; performance measurement and reporting; developing community partnerships and acts as a liaison to community agencies and other hospitals.


QUALIFICATIONS:


  • Master's Degree in Social Work required.
  • Current membership with the Ontario College of Social Workers and Social Service Workers required.
  • 5 to 7 years recent related experience in resource management, quality improvement, case management and discharge planning required.
  • Varied clinical experience across the continuum of care required, with experience in acute care preferred.
  • Demonstrated excellent verbal and written communication skills required.
  • Demonstrated strong interpersonal, negotiating, consensusbuilding and teaching skills required.
  • Demonstrated strong leadership skills required.
  • Demonstrated knowledge of network of internal and external resource contacts required.
  • Demonstrated knowledge of adult education principles required.
  • Demonstrated excellent presentation skills required.
  • Demonstrated knowledge and skill using computer office support software required.
  • Demonstrated excellent time management and organizational skills required.
  • Demonstrated willingness to learn new skills and support change management required.
  • Demonstrated basic knowledge of utilization management including CMGs, EDD, LOS, and benchmarking required.
  • Demonstrated dedication to continued education, continuous process improvement and evaluation required.
  • Demonstrated excellent critical thinking, problemsolving, decisionmaking and conflict resolution skills required.
  • Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families.
  • Gentle Persuasive Approach training preferred.
