about the role:

The System Navigator position is part of the Integrated Recovery and COVID Care Clinics in the Downtown East OHT. The Recovery Clinics increase access to preventative care, primary care, mental health, and community support services. There will also be a focus on improving access to COVID testing, vaccines and anti-viral. The Integrated Recovery and COVID Care Clinics are in partnership with Regent Park Health Centre


day to day responsibilities:


  • Conduct intake assessments and work closely with the clinical teams, ambassadors, Health Access Team, Community Health Teams to coordinate client appointments and provide information, services and programs.
  • Refer clients appropriately to services, and through a warm transfer whenever possible
  • Maintain accuracy of client data, data entry and documentation
  • Assist and advocate to help clients navigate and access community resources and services and ensure that clients' needs are met.
  • Ensure appropriate transfer to case management services as required.
  • Support Integrated Recovery and COVID Care Clinics planning and implementation
  • Develop and maintain cooperative relationships with partner agencies and other service providers providing programs and services for the populations served.
  • Maintain regular contact and communication with the Health Access Team and the team at Regent Park and support planning and case consultation around developing a plan of action for engagement with the clients accessing the clinics
  • Attend various programs and outreach events in the St. James Town and Regent Park communities
  • Maintain communication and contact with local serviceproviding organization staff and follow up with warm transfers and referrals
  • Record client feedback on service navigation satisfaction by contacting clients within a month of receiving services
  • Demonstrate a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with SHCrelated policies, health and safety legislation and best practices
  • Other duties as required

To thrive in this role, you'll need

education/experience/skills/abilities:


  • Uptodate knowledge of the community resources in the Downtown East
  • Understanding of the local community and populations in the DET OHT neighbourhoods and familiarity with health and social issues in the community
  • Knowledge and familiarity in using social media for outreach
  • Undergraduate degree in the health or social sciences or equivalent of experience and expertise.
  • Experience working in urban, diverse neighbourhoods
  • Experience working with highneed, hardtoreach clients and communities
  • Familiar with Data Collection
  • A second language is an asset.
