Primary Team Leader Responsibilities:


  • In collaboration with the team and manager, organize and prioritize daytoday operations of the team, staffing, scheduling and resource utilization
  • Round, monitor and address patient and family satisfaction with care
  • Facilitate staff orientation and identify staff learning needs and appropriate resources in collaboration with program Educators & Manager
  • Support the accountability of team members to the Manager with regard to patient care, professional conduct and performance
  • Participate in hiring, orientation and performance evaluation of staff
  • Liaise with members of the interprofessional team (inpatient, outpatient and Emergency Department) to ensure excellence in patient care
  • Regularly completes chart reviews, audits and followups to ensure adherence and compliance to standards of care
  • Assists in the development of effective interpersonal relationships among the interprofessional team through coaching, mentoring and facilitating difficult conversations
  • Support patient flow, discharges and transfers of patients through a transparent, equitable and standardized process

_ In addition to the above, this position will hold a reduced allocation of clinical time._

Primary Clinician Responsibilities

  • Responsible and accountable for decisions and actions in the provision and management of patient care.
  • Coordinating follow up services with relevant community resources
  • Practitioner, consultant and educator for appropriate patient groups and health care team members
  • Provide written assessment, treatment recommendations, progress notes and discharge summaries
  • Provide clinical teaching, mentoring and/or supervision for Social Work students; engaging with interprofessional education experiences with students of all health disciplines

Key Qualifications

  • Minimum five years recent experience, with adults living with Addictions and Concurrent Disorders
  • 3 years of experience in a
    formal or informal leadership position within a healthcare environment.
  • Masters' Degree in Social Work, Nursing, Counselling Psychology, Occupational Therapy (or equivalent) in good standing with the Regulatory College
  • Previous clinical mental health experience in an acute care hospital setting is required
  • Sound knowledge of mental health disorders and pertinent legislation
  • Sound knowledge of mental status evaluations, psychosocial assessments, suicide and violence risk assessments, psychiatric history taking, and discharge planning
  • Comfortable working within a multidisciplinary team, under pressure and within a changing environment
  • Skilled working with a broad range of client groups and their significant supports
  • Able to maintain timely and thorough documentation
  • Working knowledge of community resources
  • Excellent interpersonal, communication and organizational skills
  • Demonstrated awareness and a good understanding of the principles/protocols that govern health and safety issues
