Your Opportunity:
Brooks LTC/Sub-Acute care is a combined 24 bed nursing unit providing care to 15 LTC residents and 9 transition patients who are either receiving rehabilitation services to return home or transitioning to a new living environment.
As a member of the CoAct team, the Health Care Aide provides resident and patient centered care according to AHS policies and Continuing Care Health Service Standards.
You will collaborate with the interdisciplinary team, resident/patient and their families, providing wholistic care, by supporting residents and patients with activities of daily living while optimizing their independence.
You will complete the resident's scheduled RAI MDS tracking tool with the team and participate in the development of the resident's care plan.
You will observe, monitor and report any changes in the patients/residents physical, mental, and emotional state to the Registered Nurse.

As a Health Care Aide (HCA), you are responsible for providing personal assistance, supporting activities of daily living and providing comfort and support services to patients (clients, residents) who require short-term assistance or ongoing support.
You will work closely with patients, families, and caregivers spanning the continuum of care in home, community or health care facilities.
You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
