Position::Personal Support Worker - Home Support and Day Program (Part-time)


Hours of Work::Variable based on client demand


Reports to::Home Support Supervisor

Position Summary:


Responsible to provide personal support services to enable client seniors to continue living with independence, and to be healthy and safe-at-home.

Ensures satisfaction with the services by establishing and maintaining relationships with client seniors and their care providers and/or family members.

Delivers personal support services in both of SPLC's Day Programs and Home Support Services environments according to established care plans


Responsibilities:


1.

Provides personal support services to community Clients, as directed by the service and/or care plan and following procedures in the Home Support Services Handbook
:


  • Cues and prompts Client to participate in and be independent as possible with their own activities of daily living
  • Provides personal care related activities of daily living: bathing, washroom assistance, changing incontinence products, dressing, personal hygiene and grooming and dental care
  • Assists with transferring and positioning of Client
  • Assists with meal preparation, setup and eating
  • Assist Client with exercise programs under the direction of a health care professional including walking with the Client
  • Provides medication reminding to the Clients (e.g. blister pack)
  • Reports any incidents to Care Manager and/or Supervisor and follows procedures in the Home Support Services handbook
  • Obtains approval from Supervisor for any changes to the work schedule
  • Promotes an inclusive environment for Clients respecting abilities, age, ethnicities, languages, genders, sexual orientations

2.

Provides Home Making services to community Clients:


  • Provides light housekeeping services to community Clients
  • Completes household tasks according to the Home Making Components and the care plan

3.

Provides personal support services to Clients and daily activities in the Day Program, per the service and/or care plan and the procedures in the Day Program Handbook
:


  • Assists program activity implementation and encourages Client participation in activities with both physical and verbal support as required
  • Provides personal care related activities including but not limited to: feeding, cleaningup, toileting and hygiene, medication reminders
  • Assists with cleaning and personal hygiene assistance after Client incontinence
  • Responds to medical emergencies and notifies Supervisor immediately
  • Applies proper Client support techniques to prevent Client injuries and ensure Client safety at all time
  • Escorts Clients on SPLC's vehicles during transportation to and from the program and ensures their safety while getting on and off the vehicle, during transportation and on arrival at home
  • Assist with physical set up for program activities and daily routines, including cleaning, sanitizing ensuring hygiene of program areas at all times
  • Assists Day Program staff including volunteers and placement students
  • Ensures efficient operation of the Day Program by backing up other staff when absent, and this may involve running regular Day Program activities per the Day Program schedule for short periods of time
  • Attends and participates in case conferences and other organization and department activities when needed
  • Ensures Clients' satisfaction and the department's standards and procedures are met by working cooperatively with other Team Members
  • Promotes an inclusive environment for Clients and Team Members, respecting abilities, age, ethnicities, languages, genders, sexual orientations

4.

Documents services and reports concerns and occurrences:


  • Reports to the Care Manager and/or Supervisor about any unusual occurrences such as health concerns or emergencies, Client requests for different services, etc.
  • Reports to Day Program Team Leader about changes in Day Programs Clients' social, physical and mental status
  • Completes the Client Reports for Home Support Services when there are changes in Client's physical, emotional, mental status and ongoing service requirements
  • Clock in/out and electronically documents care plan tasks with the AlayaCare Mobile app following every visit

5.

Promotes health and safety of Clients and Self:


  • Works safely at all times and ensures the safety of self, Coworkers and Clients by preventing hazards and r
