About the Opportunity:


Reporting to the Manager of Patient Flow and Medical Social Work, the Medical Social Worker will provide professional social work services to patients and their families.

Services may also include the provision of support groups.

With considerable latitude and encouragement to initiate and work independently, the incumbent will be an adaptable and flexible person with the ability to work collaboratively in a variety of work settings.

Social Workers are expected to exhibit sound professional judgement and initiative while working within the framework and objectives of Nova Scotia Health.

Some home visits and travel are required.


About You:

We would love to hear from you if you have the following:

  • Master's Degree in Social Work
  • Registered or eligible for registration with the Nova Scotia College of Social Workers (NSCSW)
  • Minimum of two (2) years clinical experience required
  • Evidence of extensive knowledge of community resources
  • Evidence of ability to work independently with a multidisciplinary team as well as with both hospital and communitybased services/agencies
  • Evidence of ability to coordinate/develop/document and implement care plan for hospital discharge and safe transition back to the community or institutional setting
  • Knowledge of Joint Discharge Operations Committee and its purpose
  • Competencies in other languages an asset, French preferred

Hours:


  • Permanent, Fulltime / 100% FTE / 75 Hours Biweekly
  • Travel required (50%) between the three sites
  • Victoria County Hospital, Buchanan Hospital, and Northside General Hospital

Compensation and Incentives:

$ $49.05 Hourly
