SUMMARY OF POSITION:
Responsible for the provision of recreation and leisure programming which allows each resident an opportunity to achieve their highest potential utilizing the Therapeutic Recreation Standards of Practice as outlined by Therapeutic Recreation Ontario and the Canadian Therapeutic Recreation Association.
- Initial and ongoing assessment of each resident's physical, spiritual and leisure needs and interests through an individualized and systematic process which determines each resident's plan of care and appropriate interventions and which adheres to the MOH LTC Act 2007.
- Development of an individualized treatment plan for each resident within designated caseload. Utilization of assessment data as well as collaboration with the multidisciplinary team.
- Development of detailed outcome focused program outlines which reflect current leisure needs and interests of residents, including purpose, environment, program description, target population, goals, outcomes, resources, risk factors and evaluation.
- Provision of program delivery which is outcome oriented and directly reflects the assessed plan of care of individual and collective residents. Program delivery can encompass individual and group programs and utilize the continuum of care model.
- Comprehensive documentation related to individualized recreation intervention, assessment and evaluation through written, verbal and electronic means through Point Click Care, MDSRAI, Leisure Assessment and Statistical Software Activity Pro.
- Ensure ongoing assessment of programs and service utilizing resident, family and staff feedback.
- Ensure residents have an opportunity for community involvement both through both internal community events and external community out trips.
- Works in a manner and with the protective devices, measures and procedures required by the County of Elgin. Ministry of Labour, Ministry of Health and Occupational Health and Safety Act Regulations.
- Advises immediate supervisor of the existence of any potential or actual danger to the health and safety of a worker of which he or she is aware.
- Acts as liaison with affiliating Student Placements such as Co-operative Education, College, University programs, research projects as well as registered and community volunteers.
- Provides ongoing education and support to staff where required, to ensure the profession of recreation and leisure is promoted within the County of Elgin Homes.
- Submits to the Manager of Programs and Therapy Services the need for supplies and equipment based on an approved annual budget.
- To work collaboratively with the multidisciplinary team to ensure restorative care programming is provided for residents within the County of Elgin Homes.
- Follow specific guidelines, which may relate to leisure interventions as designated and signed by the Medical Director or designate.
- Demonstrate an interest in assuring current practices are followed in relation to Long-Term Care and recreation service delivery. This would include participation in external educational opportunities, reading of related journals and attendance at continuing education sessions.
QUALIFICATIONS:
- Completion of a recognized post-secondary degree / diploma at the University or Community College level in Recreation and Leisure Studies or other allied Health Profession.
- One to three years experience in geriatric setting or equivalent of qualifications and experience.
- Working knowledge of Therapeutic Recreation Ontario Standards of Practice.
CONDITIONS OF EMPLOYMENT:
Must be available and willing to work scheduled shifts, call-ins, weekends, evenings, and nights - seven (7) days a week.
Satisfactory TB documentation (completed within the past 12 months)
BENEFITS & VACATION
Will receive 14% in-lieu of benefits paid every pay, however, if enrolled in the OMERS pension plan, your in-lieu of benefits will reduce to 9%
4% vacation pay, paid every pay
