Aatikah Azeezmohideen

I graduated from George Brown College with a diploma in health information management. I am also certified by the Canadian Health Information Management Association (CHIMA). My previous experience with UHN includes a research project for patients with Non-Hodgkins Lymphoma. I worked with various healthcare systems such as the Electronic Patient Record and Epic to create a database for patients with CAR-T therapy. I have extensive knowledge and experience with Folio Views (ICD-10-CA, CCI), Comprehensive Ambulatory Classification Systems (CACS), and privacy laws (PHIPA, Public Hospital Act, etc). I am also skilled at working independently with minimal supervision, and working as a team.

Summer Student- BMT- IEC Data Coordinator                                         June 2022- November 2022    Created a database for patients who were diagnosed with Non-Hodgkin’s Lymphoma and were treated with CAR-T cell therapy. Utilized the Electronic Patient Record and Epic to access health files and document demographic information pertaining to the patient. Scheduled meetings for staff to review and evaluate the progress of the project and presented key findings. Monitored and updated the status of the patient’s information which was critical to the outcome of the research project. Performed various clerical duties including typing correspondence, processing applications, etc. Support preparation and uploading of medical documentation to database. Assist staff with follow-up for any missing information. Responded to emails and reported any issues faced with the project to staff.

Student Intern                                                                                                  March 2022- May 2022

Completed wait time surgical listing project that lists over three thousand operational procedures. Assigned the correct CCI (Canadian Classification of Health Interventions) codes to match the description of the procedure. Reviewed the Canadian Coding Standards for Version 2022 and created a PowerPoint to describe those standards that are new, retired and amended. Attended every meeting on time to discuss the assigned task and reported any concerns regarding the projects while determining what will be accomplished next. Met project deadlines by working effectively under minimal supervision and assisting peers in addressing any issues or concerns regarding the tasks assigned.
