LONG TERM CARE/NURSING HOME:

• Assisting residents on their activities of daily living such as bathing, eating, oral hygiene, and personal hygiene.
• Transferring resident from bed to wheelchair, and vice versa using appropriate equipment and proper body mechanics.
• Appropriately repositioning bed bound residents
• Providing safety to high risk fall resident by assisting them at all times
• Monitoring and assessing the conditions of the resident on daily basis
• Reporting to nurse in charge any unusual signs and symptoms present to resident
• Applying topical medications to the patients
• Giving feeds to Naso-Gastric tubes patients 
• Insertion and giving feeds to Naso-Gastric tubes patients 
• Performing wound dressing and applying appropriate medication
• Taking vital signs and monitoring for any signs of abnormalities
• Taking blood glucose level using blood glucose monitoring machine
• Ensures proper handling of equipment and maintenance by reporting if it is functioning or not.
• Proper documentation

HOSPITAL:

• Head to toe assessment upon arrival of patient
• Taking vital signs and monitoring for any signs of abnormalities
• Planning nursing care for individual patient
• Performs insertion of IV fluids and medications
• Performs ECG and record it’s result.
• Administering oral medications for patient and monitors any unusual reaction of the medicine
• Writing proper documentation for each patient
• Carrying out Doctor’s order
• Performs insertion of urinary catheter aseptically
• Assist patient for diagnostic procedures
• Ensures proper handling of hospital equipment and maintenance by reporting if it is functioning or not.
• Providing emotional support for the patient and relatives
• Providing health teaching for discharge patients and relatives
