About Kimberly McMullin:

I am a qualified Social Worker registered with the Ontario College of Social Workers and Social Service Workers (821745) and a member of the Ontario Association of Social Workers. I have a Bachelor's in Psychology and an Honours Bachelor of Social Work from Carleton University. I am currently pursuing a certificate in Disability Management at the University of Fredericton.

Throughout my social work career, I have worked in a variety of rehabilitation roles including as a Disability Case manager with Banyan Work Health Solutions, a Case manager with FunctionAbility (specializing in neurorehabilitation and complex mental health), Health Links program at Champlain LHIN and Director of Care at Spark Lifecare.

 As a skilled case manager, I am proficient at completing comprehensive psycho-social assessments with clients and families (including social data) to support social work interventions, in accordance with professional practice standards, privacy and clinical policies.  I excel at the opportunity to collaborate with multidisciplinary rehabilitation teams and preparing comprehensive documentation to support client centered rehabilitation goals, including coordination of services from hospital discharge to community-based rehabilitation settings.

With over 8 years of experience supporting clients in the community, I am well-versed in public and private community resources and enjoy the opportunity to connect with clients of all ages and backgrounds.

I believe that empathy, integrity, dignity, competence and human connection are the essential core values of the social work profession.  I am passionate about supporting clients in overcoming personal challenges through self-development and awareness, allowing them to regain control and address the oppressive forces that block them from thriving.

 

Experience

Throughout my social work career, I have worked in a variety of rehabilitation roles including as a Disability Case manager with Banyan Work Health Solutions, a Case manager with FunctionAbility (specializing in neurorehabilitation and complex mental health), Health Links program at Champlain LHIN and Director of Care at Spark Lifecare.

I am a skilled case manager and creative team leader with 8 years of social work experience in psychosocial assessment, neurorehabilitation, complex mental health, psychosocial rehabilitation and chronic pain. Acting as an advocate and liaison between clients, their rehabilitation team and the medical community in accordance with professional practice standards and privacy guidelines. Operating from a Structural Social Work perspective integrating Strength-Based practice, Trauma Informed Care, Family Systems theory, Emotion Coaching and Solutions Focused Therapy. Supporting clients and families with crisis intervention and facilitating access to community resources and services while ensuring culturally safe practices.

While Director of Care at Spark Lifecare, I was responsible for overseeing Home and Community Care including children and youth respite programs (autism, medically complex care and complex mental health), attendant care, senior care and the Passport Program for adults with developmental disabilities. Maintaining collaborative interdisciplinary relationships with referral sources and community partners to support difficult-to-service clients while ensuring client safety and the highest quality service.  Liaising with various community partners including social workers and Board Certified Behaviour Analysts at CHEO.  Providing supervision and mentorship to a team of 5 case workers including support with communication, education on social determinants of health, outcome-based client intakes and assessment, community resources and system navigation, professional boundaries, behavior management strategies, collaborative problem-solving and crisis management interventions.

In order to ensure the ongoing safety of our clients, families and caregivers, I created, implemented and maintained a companywide Covid-19 strategy in line with provincial guidelines and public health measures.
