    Description

    In this community nursing role, as a Visting Nurse you will work both on-call overnights and have scheduled visits to care for patientsin various settings (ie. home and school). You will work with patients with differing complexity of care needs and play a key role in helping patients remain safe and comfortable in their community for as long as possible.

    • What you'll do:

  • Work overnights on-call and Care for patient needs during scheduled visits in your community
  • Frequently utilized skills may include: tracheostomy care, suctioning, ventilator care and monitoring, g-tube feed and maintenance, transfers, personal care as required.
  • Execute and continuously monitor care plans
  • Engage patients, families and team members, using a collaborative approach by building rapport and trust

    • What you bring:

  • Registration with the CNO as a Nurse in good standing
  • Valid driver's license and access to a vehicle is required
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences/school settings and day programs
