We are seeking a Nurse Case Manager who is passionate about supporting people through their healthcare journeys and helping employers, insurers, and partners achieve positive outcomes.Responsibilities
- Case Management: Review medical records, assess treatment duration, and ensure care meets employees' medical needs.
- Communication: Serve as the main point of contact between employees and employers during absences; maintain accurate case notes and documentation in OHGMed.
- Assessment & Recommendations: Recommend additional assessments as needed (e.g., Ergonomic, Independent Medical Evaluations, Fitness-to-Work).
