The Espanola & Area Family Health Team is currently recruiting a Full-Time Social Worker. The successfulprimary care providers to improve the health of the patients' accessing services through the Family Health
Team.
This position offers a competitive compensation package including paid vacation and sick leave, health and
dental benefits, life insurance and pension plan. The starting wage for this position is $ per hour
depending on experience.
Responsibilities Include:
- Complete assessments and make recommendations for care of patients and families with mental health
- Provide individual and group counseling, support and treatment in accordance with medical treatment
- Prepare psychosocial case history, assessment and social work treatment plans for use in screening and
- Liaise with community partners to develop services and programs that meet the needs of the
- Develop, facilitate and evaluate workshops, programs and services.
- Participate in mobilizing resources in the community, maintains cooperative working relationships with
- Work collaboratively with other FHT staff to improve patient care, service delivery and coordination.
- Act as patient advocate to problem solve and ensure health care needs are met with internal and
- Maintain patient charts in the EMR.
- Support patients by making appropriate referrals within the Family Health Team.
- Model and promote a positive team based work environment and maintains confidentiality.
- Perform other related duties as assigned.
Qualifications:
- Current registration in good standing with the College of Social Workers of Ontario
- M.S. W. preferred or B.S.W. with 3 years clinical experience
- Demonstrated experience working in community setting or primary care setting
- Knowledge of the principles, procedures, trends of clinical social work.
- Experience with dispute resolution and crisis management
- Demonstrated ability to problem solve effectively and independently
- Demonstrated ability to work effectively within an interdisciplinary team
- Strong interpersonal and communication skills
- Possess a driver's license and reliable vehicle
- Fluency in English and French is an asset
