Temporary assignment - approximately 26 weeks

***The role of the Stay Well Program Assistant is to assist in the facilitation of group exercise/social programming as part of the Stay Well program and lead the LHIN Program at St. Joseph's Healthcare Hamilton.


QUALIFICATIONS

  • Completion of a community college diploma in Health, Wellness, Fitness or equivalent
  • Undergraduate degree in Kinesiology, Exercise Physiology, Health Sciences is preferred
  • Current Kinesiologist registration is preferred
  • Certified in Group Exercise Leadership and/or Personal Training/Individual Conditioning (CSEP preferred, OFC, CanFitPro)
  • Current certification in CPR/First Aid Level C
  • 1 year of relatable experience
  • Working with but not limited to special populations in particular frail elderly adults
  • Creating and implementing exercise programs that reflect the needs of the population
  • Working within a group setting where safety and quality of program are top priority
  • Leading and facilitating small and large groups
  • Demonstrated knowledge of various medical conditions prevalent among special populations, particularly the frail elderly. Examples can include but are not limited to arthritis, osteoporosis, Parkinson's, musculoskeletal, neurological and cognitive conditions etc.
  • Must be flexible to adjust program instruction to the level of capability of participant population while maintaining the physical and mental health objectives of the program and providing an enjoyable experience for participants
  • Demonstrated interpersonal skills that reflect the values of SJHH including dignity, respect, service, justice, responsibility and enquiry
  • Effective communication skills for providing instruction to participants
  • Ability to lead groups with consistent enthusiasm, patience and compassion

RESPONSIBILITIES

  • Leads small and large group exercise programs for special populations and in particular the frail elderly, under the guidance and as designed by the Stay Well Program Coordinator
  • Adjusts program deliverables to accommodate the physical and mental capabilities of program participants
  • Designs and lead LHIN funded exercise programs at a level appropriate for the class attendees
  • Takes lead in designing and executing the health promotion and recreational calendar of The Stay Well Program, which are planned month to month for clients
  • Meets with the Stay Well Program Coordinator to review participant progress, and provide feedback on necessary adjustments or modifications to programs to accommodate physical / mental capabilities of participants
  • Follows up on participants that were absent from the LHIN and Stay Well Program classes
  • Communicate changes in health status of Stay Well Program clients accordingly to Stay Well Program Coordinators
  • Assists the Stay Well Program Coordinator with the daily flow of the program, by assisting with program setup and cleanup, executing patient transfers, portering patients etc. as required
  • Assists the Stay Well Program Coordinator in the provision and supervision of patients' snack break and in the planning and execution of social events
  • Completes intake assessments of new clients signing up for the Stay Well Program as assigned by the Stay Well Coordinator
  • Creating promotional material such as presentation, flyers, news paper ads and other forms of media to raise awareness of the Stay Well and LHIN programs
  • Participates in all other duties as assigned by the Stay Well Program Coordinator and Program Manager
