The Middlesex London Ontario Health Team (MLOHT) is seeking a systems-thinker with quality improvement, evaluation, and analytics experience and expertise to work effectively with the MLOHT Operations Team, Population Health Lead, Quality and Analytics Co-Leads, Quality and Analytics Advisory Group, and Quality and Analytics Working Groups to lead our overall data analytics and evaluation framework.

As the MLOHT evolves toward an integrated system that supports our entire attributed population of over 500,000 people, the MLOHT Quality & Analytics Advisory Group and complementary Working Groups will work to develop and implement an evaluation strategy to ensure that we are appropriately measuring the advancement of our purpose:

The MLOHT Quality and Analytics Coordinator will engage in the ongoing co-design and implementation of our evaluation framework and performance measurement plan for the MLOHT.

This plan will be built around the principles of Population Health Management that aligns with the Middlesex London OHT's commitment to a Health Equity-Driven Quadruple Aim.

At all times, this work will be completed with our OHT Operations Team, local stakeholders (including patients/clients, caregivers/care partners, and providers), and local researchers/data analysts.


Key Responsibilities

  • Lead the development, evolution, and implementation of the MLOHT Evaluation Framework and Performance Measurement Plan
  • Lead the annual development of the MLOHT Collaborative Quality Improvement Plan (cQIP), including facilitating the collective identification of key change initiatives, associated targets, and lead partner organizations
  • Coordinate everyday tasks and activities related to quality improvement initiatives across the MLOHT, including maintaining communication with MLOHT Leads, Quality and Analytics coLeads/Chairs, and key stakeholders
  • Provide evaluation and performance management expertise and guidance to MLOHT projects and initiatives
  • Actively participate in local, regional, and provincial Evaluation and Performance learning opportunities, to stay informed of evolving data sources and approaches available to OHTs
  • Envision/design systemlevel Population Health Management information strategies and, where possible, perform systemlevel analytics and create reports and summaries
  • Act as a liaison for the overall Quality and Analytics Team/Working Groups
  • Work with MLOHT system partners to facilitate data collection (from people accessing health care, providers, caregivers/ care partners, and others in the community as needed), storage, transfer, use, and governance.
  • Drive ongoing review of Analytics Releases to ensure it is fit for purpose and continuously improved across the OHT

Key Competencies

  • Passion for and experience in Quality Improvement (QI), evaluation, data and analytics, measurement, and reporting.
  • Expertise, understanding, and/or passion for population health management.
  • Selfdirected and highly motivated with superb interpersonal and communication skills (both written and oral).
  • Demonstrated ability to motivate/guide/coach/influence/facilitate collaborative discussions with multiple stakeholders.
  • Demonstrated innovative thinking and ability to handle and navigate ambiguity.
  • Relentlessly focused on ensuring patient/client, caregivers/care partners, and providers have a central role in design, planning and decision making.
  • Proven ability to build, strengthen, and maintain relationships and partnerships with stakeholders from diverse sectors, organizations, and experiences.
  • Proven ability in evidencebased performance measurement planning relevant to our local context.

Qualifications and Experience

  • University degree or equivalent training/experience in Epidemiology, Biostatistics, Computer Science, Mathematics, Business Information Technology, Information Management, or other related field.
  • Preferred five years of experience in health care improvement with a proven ability to work with diverse stakeholders, including frontline providers, management, and senior business leaders
  • Preferred five years of experience in applied health system measurement and evaluation.
  • Experience in communicating and presenting solutions to technical and nontechnical audiences, and to all levels of management, including senior executives.
  • Awareness of data sources for healthcare measures and ability to engage partners to define and collect data across multiple data sources.
  • Experience in Project Management an asset.


This is a hybrid role, combining both in-office and remote work, with occasional travel required throughout the region as needed.
