Job summary
The Home Health Registered Nurse (HH RN) utilizes the nursing process to coordinate the interdisciplinary approach to patient care for her/his team of patients.
Responsibilities And Essential Job Functions
- Demonstrates effective clinical critical thinking skills as demonstrated in application of disease management protocols, pain management interventions and clinical equipment according to department processes and policies.
- Demonstrates effective management of patient care as evident in patient Plan of Care, medication management, patient teaching and use of visit frequency.
