Home and Community Care Support Services South East is one of 14 Home and Community Care Support Services organizations in Ontario with a focused mandate to deliver local health care services such as home and community care and long-term care home placement.


These organizations were previously known as Local Health Integration Networks (or "LHINs") at a time when they also led local health care planning and funding, and those functions are now part of Ontario Health.


Home and Community Care Support Services South East is dedicated to ensuring the ongoing delivery of local services while Ontario makes changes to improve the health care system to give patients better-connected care with health care providers working as one coordinated team in Ontario Health Teams.

The south east region extends from Brighton on the west, to Prescott and Cardinal on the east, north to Perth and Smiths Falls, and back to Bancroft.

The boundaries are for funding and planning purposes only.

Residents of the Home and Community Care Support Services South East can seek health care services wherever they prefer, inside or outside these boundaries.

Home and Community Care Support Services South East is looking for a Community Relations Coordinator to help maintain our online database with nearly 3,000 local service listings.


Supporting the Senior Manager, Communications, the Community Relations Coordinator is responsible for ensuring that service listings are easily accessible, reliable and up to date in order to help connect people with health service supports available throughout the south east region.

What must you have?

  • Postsecondary degree or diploma in Public Relations, Communications, Journalism or Marketing and/or Information Science or equivalent combination of education and experience
  • 3 + years' progressive experience in information management and working knowledge of database management practices and web publishing, preferably in a health care setting
  • Valid Ontario driver's license and access to insured reliable vehicle required
  • Competence in desktop publishing and web maintenance software are an asset
  • Information and Referral (I&R) designation an asset

What will you do?
Development and maintenance of

  • Administer , our information and referral database, including overall site maintenance, upgrades, recording updates and providing guidance on navigating, searching and leveraging
  • Lead database expansion opportunities by making contact with, obtaining authorization, setting up user accounts, verifying information entered by organizations into the database and entering agency profile information
  • Provide orientation and training on information and referral to Home and Community Care Support Services South East Access Centre staff.
  • Assist staff with information needs as requested, including supporting other areas of Home and Community Care Support Services as a data/content liaison for special projects
  • Work with the provincial team to develop mini
- and micro-sites for , including maintenance, upgrades and analytics reporting

  • Serve as the Home and Community Care Support Services South East representative for provincial working groups
  • Develop feedback tools, monitoring website usage and providing statistics for management reporting and in order to identify recommended enhancements and/or regular modifications
