Program Summary:

The Community Treatment Orders (C.T.O.) Case Manager R.P.N.

is an experienced mental health clinician providing registered practical nursing and case management services to clients on Community Treatment Orders in the North East Cluster of the Central East Local Health Integration Network, including the counties of Haliburton, Peterborough, Northumberland, and the City of Kawartha Lakes.


The purpose of this role is to promote stabilization and recovery in the community, as well as independent living and prevention of unscheduled return visits to hospital.

This includes responsibility for assessment, providing individualized support and intervention to clients within the community, ongoing individual service planning and evaluation, consultation with other service providers and crisis intervention.

The Community Treatment Orders (C.T.O.) Case Manager R.P.N. will ensure that the delivery of service, coordination, linkages and advocacy are provided as required. This position will work in close collaboration in a team model with the other R.P.N.s, the regional C.T.O. Coordinator, hospital staff, primary care and any other members of clients' circles of care.

Vaccination Policy:

C.M.H.A. H.K.P.R.

will require all new staff to be fully vaccinated against COVID-19 and provide proof of vaccination status before the first day of employment.

For this purpose, an individual is defined as fully immunized equal or greater than 14 days after receiving their second dose of a two-dose COVID-19 vaccine series or their first dose of a one-dose COVID-19 vaccine series that is approved by Health Canada.


Key Responsibilities:


1.
Client Services

  • Provide overall case coordination for clients, in fulfillment of responsibilities outlined on each client's Community Treatment Orders (C.T.O.).
  • Complete evidence based Canadian Mental Health Association service plans and crisis plans, in keeping with the C.T.O.
  • Ongoing assessment, monitoring and supporting clients working towards treatment goals.
  • Perform nursing duties with clients, including medication administration, long acting injections, monitoring of medication efficacy and sideeffects.
  • Perform mental health status assessments, where applicable and collaborate with psychiatrists within the parameters of the C.T.O.
  • Assist individuals with system navigation.
  • Transport/accompany clients to appointments as required.
  • Provide crisis intervention, advocacy when requested.
  • Provide referrals to internal and external services.
  • Work in close collaboration with C.T.O. team members
  • Provide information about mental health to clients, family members, and other service providers

Documentation

  • Complete all required documentation and statistical activities for C.M.H.A. H.K.P.R.
  • Accurate and timely completion of all other administrative documentation including time sheets, expense claims, vacation and timeoff requests, and training requests.
  • Accurate and timely completion of progress notes, as per agency policy, on client record, in conjunction with data collection processes of service delivery partners, Nursing Assessments, metabolic monitoring, Medication Reconciliation and Best Possible Medication History summaries.
  • Participate in completion of O.C.A.N. assessments as appropriate.

3.
Other Activities

  • Develop and promote partnerships with clients, families, service providers, agencies, community partners, hospitals and police.
  • Participate in community outreach activities through the development of collaborative partnerships and through presentations, education and visits to community agencies and services as appropriate.
  • On an occasional rotational basis, fill in for other Nurses in other C.M.H.A. H.K.P.R. programs, during periods of absence, to ensure service continuation.

4.
Performance Management and Professional Development

  • Participate cooperatively in the agency performance management process/system.
  • Stay current on relevant legislation and best practices, current research, policies and procedures.
  • Participate in ongoing professional development and mandatory training.
  • Actively engage and participate in supervisory process.
  • Maintains current certificate of competence as an R.P.N. wit
