Affiliated Services for Children and Youth (ASCY) requires a Family Support Worker.for a
12 month contract. The Family Support Worker is an integral member of the Infant Hearing Program (IHP) and Blind-Low Vision Program (B-LV) and is accountable to the IHP Program Manager in conjunction with the Director of Child and Family Services, and the Executive Director of ASCY.

Reports to:
Infant Hearing Program Manager and Director of Child and Family Services
Location: 26 Arrowsmith Road, Unit 1, Hamilton, ON
Purpose:
This position will provide support to the IH and BL-V programs
What we offer:
- 12 month contract position of 37.5 hours a week
- Monday
- Friday, 8:30 4:30 with flexibility to occasionally work outside of regular hours
- Salary: $36.25$43.25/hr. depending on experience
- 8% vacation pay
- 3 days of paid time off during December holiday season
Clinical Responsibilities:
- Receive referrals of babies with permanent hearing loss from the audiologist and complete intake
- Ensure the family receives up to date, unbiased, information concerning options for communication development
- Facilitate referrals to community professionals and community resources as required
- Hold and document regular Communication Development Plan meetings and other documentation required by the funder
- Support Music therapy programs with a Registered Music Therapist
- Provide parent education/support workshops
- Provide guidance/work collaboratively with the Parent Mentor
- Work with BLV Program consultants and provide family support to clients in that program
- Liaise with other community professionals involved with the family (e.g. NPCC, NICU, Infant Parent Program, McMaster Children's Hospital social workers and other team members)
- Provide regular followup to ensure family needs are being addressed
- Responsible for any and all other duties as may be required in accordance with proper training and certification requirements
Administrative Responsibilities:
- Support families in accessing community resources and financial supports
- Assist with documentation and the completion of entry and reporting requirements for the Healthy Child Development
- Integrated Services for Children Information System (HCD-ISCIS)
- Maintain clinical records using the Electronic Health Record
- Communicate regularly with the Program Manager regarding families in crisis and program issues, reporting on caseload and professional progress with each
- Support virtual and inperson services
- Responsible for any and all other duties as may be required in accordance with proper training and certification requirements
Qualifications:
- BSW or MSW or education in a related field
- Registered with Ontario College of Social Workers and Social Service Workers
- 4 years of experience
- Experience working with families with children with special needs
- Leadership skills
- Excellent interpersonal abilities
- Excellent written and oral communication skills
- Ability to work as part of a team
- Ability to regularily travel to locations in Hamilton, Niagara, Brant and Haldimand-Norfolk counties
- Valid Ontario driver's license and access to a vehicle
