Role Includes:

Provide direct patient care as outlined in the patient's care plan including but not limited to administration of medication, completion of treatments, and patient teaching related to care needs.
Monitor, assess, and document changes in the patient's condition
Assist with activities of daily living
Collaborate with the interprofessional healthcare team to determine and implement the patient plan of care
Additional opportunities as assigned by Nursing Supervisor
Practice within the scope of practice set out by the College of Nurses of Ontario (CNO)

Requirements:

Graduate of an accredited nursing program (Practical Nursing Diploma, Bachelors of Science in Nursing)
Current registration with the College of Nurses, in good standing
Participation in onsite training to support patient care needs with equipment including gtube feeds, tracheostomy/oral suction machine, ventilator monitoring
Valid work/study permit (if applicable)
Current CPR/First Aid Certification
Vulnerable Sector Screening (within 3 months of hire)
Uptodate Immunization Records (MMR, TDAP, Varicella, Hep B, Covid19)
Strong documentation skills
Strong communication and problem solving skills
Ability to follow client specific care plan
Reliable transportation
