Role Includes:


  • Provide direct patient care as outlined in the patient's care plan including but not limited to administration of medication, completion of treatments, and patient teaching related to care needs.
  • Monitor, assess, and document changes in the patient's condition
  • Assist with activities of daily living
  • Collaborate with the interprofessional healthcare team to determine and implement the patient plan of care
  • Additional opportunities as assigned by Nursing Supervisor
  • Practice within the scope of practice set out by the College of Nurses of Ontario (CNO)

Requirements:


  • Graduate of an accredited nursing program (Practical Nursing Diploma, Bachelors of Science in Nursing)
  • Current registration with the College of Nurses, in good standing
  • Participation in onsite training to support patient care needs with equipment including gtube feeds, tracheostomy/oral suction machine, ventilator monitoring
  • Valid work/study permit (if applicable)
  • Current CPR/First Aid Certification
  • Vulnerable Sector Screening (within 3 months of hire)
  • Uptodate Immunization Records (MMR, TDAP, Varicella, Hep B, Covid19)
  • Strong documentation skills
  • Strong communication and problem solving skills
  • Ability to follow client specific care plan
  • Reliable transportation
