As a Patient Care Contracts Specialist (FMHC), you will be responsible for the development, delivery and maintenance of all Family Managed Home Care (FMHC) agreements with Patients and/or Substitute Decision Makers (SDMs).

This position tracks and manages FMHC contractual issues, responds to questions from patients or SDMs, and works in collaboration with finance and patient services to onboard new patients, support ongoing operations and transition patients off the program as required.

What will you do?

  • During the onboarding process, meets with FMHC patient/SDM to review FMHC agreement requirements, as well as roles and responsibilities
  • Obtains applicable approvals, prepares agreements, issues contracts to patients/SDMs for signatures, tracks the submission of required documentation (i.e. Banking, insurance etc.), tracks and manages contract renewals
  • Addresses or escalates contract compliance issues to management; Recommends next steps
  • Reviews and issues to patient/SDM updated care plans and budgets based on Care Coordinator's reassessment.
  • Manages other FMHC contracts related requirements and projects as required
  • Works with FMHC Team to analyze program trends, waitlist strategies, efficiencies of process and opportunities for program improvement
  • Provides crosscoverage or backfilling to the Patient Care Contracts Team
  • Undertakes special projects as assigned that are in accordance with job responsibilities and/or necessary objectives
  • Manages confidential information in a responsible manner.
What must you have?

  • Relevant undergraduate degree or diploma in business or a health related field from a recognized college or university. Combination of relevant education and/or experience will be considered.
  • Experience and skills related to negotiation, problem solving and conflict resolution
  • Experience dealing with legal documents and contracts
  • Experience with contract administration in the public sector
  • Awareness of contract laws and legal issues and difficulties which can arise in administering Service Provider contracts
  • Experience working directly with patients and families
  • Excellent oral and written communication skills with a sound knowledge of the English language, spelling, punctuation and grammar
  • Strong negotiation and conflict resolution skills
  • Strong collaborative skills to work with patients/SDMs, and staff from various departments of Home and Community Care Support Services
  • Excellent interpersonal skills to work effectively with both internal and external stakeholders
  • Exercises tact and discretion in handling sensitive and confidential information and issues
  • We have a mandatory COVID19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID19 vaccination status prior to start date
