What will you do?
Home and Community Care Support Services is looking for a dynamic individual to support the daily operational delivery of patient service programs.
Reporting to the Director, Home and Community Care, this position will initially be assigned to lead and support a team of staff in the coordination and patient-specific ordering of medical supplies and equipment, in addition to other programs/services.
The Manager, Patient Services provides leadership, direction and operational oversight to ensure high quality patient services are provided.KEY ACCOUNTABILITIES:
- Responsible for management and distribution of Home and Community Support Services North East medical supplies and equipment for patient care;
- Collaborates and provides support to Home & Community Care and Service Providers with critical service issues related to supplies and equipment orders;
- Leads multiple projects or programs related to medical supplies and equipment including the planning, management, and implementation of the assigned project;
- Develops and maintains various policies and procedures;
- Establishes and maintains effective relationships with all key stakeholders to aid in the delivery of quality patient care;
- Leads or represents Home and Community Support Services North East on multidisciplinary committees as well as interagency and provincial working groups;
- Acts as a key clinical resource to all Home and Community Care Support Services North East staff and managers regarding owned medical supplies and equipment;
- Continuously improves quality and efficiency of current processes, patient experiences, and other strategic priorities within the portfolio;
- Contributes to a safe and healthy environment by following safe work procedures, reporting injuries, illnesses, and unsafe working conditions;
- Ensures staff engagement and involvement for change initiatives;
- Performs other duties as assigned.
POSITION REQUIREMENTS:
- Registered Nurse in good standing with the College of Nurses of Ontario;
- Significant experience in diverse and unionized community health care environments;
- Leadership experience an asset;
- Project management experience as asset;
- Familiarity with patient needs for various medical supplies and equipment;
- Working knowledge of community resources and support programs such as NIHB, ADP, ODSP;
- Welldeveloped planning, organizing, critical thinking, decision making and evaluation skills;
- Effective communication and interpersonal skills;
- Strong coaching, negotiation and conflict resolution skills in order to find resolutions especially with physicians and clinical leaders;
- Knowledge of quality improvement practices and models such as LEAN is an asset;
- Considerable hands on knowledge of MS Office including Outlook, Access, Excel, PowerPoint, and Word as well as asset/inventory management software;
- A valid driver's licence and access to a reliable vehicle;
- Advanced oral and written proficiency in English is essential, with proficiency in French considered an asset.
