Position summary
The Caseworker engages with guests and community members of St.

Felix Centre respite program to provide individualized and flexible support for a range of services and referrals including mental and physical health, housing, legal, financial, crisis management, harm reduction, etc.

As part of the Casework and Social Supports team, the Caseworker will consult with relevant stakeholders to draft, update, modify, or reassess case plans and intervention strategies, coordinate and participate in case management, consultation, and departmental planning activities.

This role will also enhance the capacity of internal and external stakeholders to address case plan related issues.


Education

  • BSW, SSW, PSW, Health Promotion or another health related field of study or demonstrated experience in a related field;
  • Crisis Prevention and Intervention certificate;
  • CPR, First aid certificate;
  • Lived experience will be considered an asset;
  • Demonstrated reliability and availability to attend scheduled shifts;
  • Ability to set expectations, and monitor progress of processes.

EXPERIENCE:


  • Minimum 1 years' experience working in a lowbarrier setting and working with diverse
- populations including people experiencing homelessness, substance use, crisis, trauma, poverty,
- and mental illness;

  • Familiarity with the "Housing First" model is considered an asset;
  • Experience facilitating partnerships with external agencies;
  • Minimum 1 year experience in case documenting in a social service setting;
  • Demonstrated ability to facilitate workshops and lead training;
  • Demonstrated reliability and availability to attend scheduled shifts.
  • Experience with coaching and employee development considered an asset

RESPONSIBILITES

  • Work with individuals in order to identify housing needs and eligibility and develop long and short term objectives designed to meet those goals;
  • Advocate for the individual and facilitate navigation of housing programs and policies to assist them in overcoming systemic barriers that may preclude them obtaining housing;
  • With the individuals consent, consult with relevant organizations, caregivers and other significant people in their lives in order to update, modify, or reassess case plans and intervention strategies;
  • Maintain and model appropriate personal boundaries with guests, and refrain from involvement with guests' personal affairs or other business that may constitute a conflict of interest;
  • Develop and implement guest focused programming and projects;
  • Support the operation of the Overdose Prevention Space and provide health and harm reduction services as needed;
  • Flexible to attend meetings scheduled outside of working hours as needed;
  • Crisis prevention, mediation, deescalation and/or coordination of services, including supportive debriefing, and assistance in the development of a restorative plan;
  • Facilitating debriefing, which includes communicating with guests and staff after a crisis has occurred;
  • Collecting and analyzing data related to restrictions with a goal of reducing restrictions;
  • Collaborating to create alternatives to restrictions and implementing these alternatives;
  • Develop and implement goals and objectives that are outcome based and measurable, and which reflect the mission of the organization;
  • Accompany guests to appointments, housing viewings, and meetings with other agencies when necessary;
  • Enhance the capacity of internal and external stakeholders by providing timely feedback and coaching to support case planning and guest engagement;
  • Coordinate and participate in case management, consultation, and departmental planning activities;
  • Understand and use correct reporting procedures for serious or unusual incidents, including but not limited to guest injury or illness, incidents of aggression, or injury involving employees;
  • Maintain positive relationships with other service providers, including but not limited to social workers, medical staff, corrections officers, and TPS;
  • Oversee and assist with the planning and implementation of program enhancements;
  • Compile caseload statistics and prepare program and guest reports;
  • Communicate effectively and use a professional approach with all team members, managers, supervisors, guests, and stakeholders;
  • Maintain knowledge of external regulatory bodies governing the organization;
  • Comply with company policies, procedures, and ethical standards within the scope of case management;
  • Identify and pursue professional development opportunities;
  • Maintain current credentials as required for employment;
  • Ensure Health and safety protocols and the use of personal protective equipment are maintained;
