    Reporting to the Manager, the Care Coordinator is a key member of the interdisciplinary team.

    Using clinical triage protocols, technology platforms, critical thinking, communication tools and other resources, the triage nurse identifies the most appropriate care path for patients of the program.

    Assisting the physician in the clinical setting can be the main responsibility when in a remote clinical setting.

    ROLES AND RESPONSIBILITIESEnsure patient records contain all relevant medical information;Provide clinical care to individuals or defined patient populations through the effective application of clinical judgement, computer-based decision support software, appropriate evidence-based clinical guidelines and other relevant information databases/resources.

    Using in-depth clinical knowledge, apply process to identify patient needs, risk and safety issues, educational opportunities and appropriate care pathway;Demonstrate compliance with all program specific performance metrics, participate in the evaluation and/or measurement of these metrics, and plays an active role in improving personal, team and program performanceUses expert knowledge and professional judgment to assess the patient's chief complaint/concern and provides approved interventions based on caller needs;Review care path recommendation with the patient that are congruent with evidence base practiceBuilds credible and therapeutic relationship with the patient by communicating effectively, articulating information clearly, and using appropriate language, clarifying the message to the receiver, validating understanding and demonstrating cultural competence.

    Process calls and online inquires based on NBHL's Policies and Procedures.
    Adheres to processes to provide service in a care effective and time efficient manner for all patients.

    Identify and assess appropriate patients for alternate services and communicate information to service provider;Maintain effective working relationships with physicians and nurse practitioners, hospital unit managers and staff, community agencies and partners;Promote and communicate the care plan goals to the NBHL Interdisciplinary Health Care Team;Lead and participate in patient care conferences to help coordinate services and share information related to known and potential NBHL patients.

    Coordinate care planning with patients, caregivers and healthcare team, including physicians and nurse practitioners, to facilitate Primary careSuch other functions as may be assigned by the Director or Manager, which are consistent with the role of the Triage Nurse;Assist in orientation/education of new physicians/nurse practitioners/community partners and new staff regarding the NBHL programREQUIRED QUALIFICATIONS AND OTHER JOB REQUIREMENTS:
    Registered in good standing with the Nurses Association of New Brunswick;Three (3) year's experience in acute care setting.

    Other nursing lead specialty clinic experience may be consideredDemonstrated commitment in achieve goals and objectives by using transformational leadership skills in all interactions.

    Have sound analytical skills and clinical judgment;Ability to work independently with little supervision.
    Ability to work in a collaboration with all stakeholders and partners in a respectful and professional manner.
    Demonstrated knowledge of current health trends and principles of primary and virtual care,Demonstrate excellent communication. The employer may require a language proficiency test.

    Demonstrate ability to use and apply a clinical structure approach in problem solving, including the use of critical judgment and analysis of clinical presentation for safe and efficient patient care.

    Demonstrated ability to use computer and various software applications including Microsoft Office;Knowledge of community resources;BCLS Certification and maintain competence;Ability to work in a remote working environment;We would like to thank all candidates for expressing interest.
