75 Bi-weekly Hours - Day-Evening including some weekends

Baycrest has been appointed as the health service provider accountable for leading the implementation of the Toronto Central regions' Behaviour Support Action Plan.
A key component of the plan is the deployment of a Behaviour Support Outreach Team ('BSOT'), consisting of interdisciplinary community outreach clinicians, nurses and personal support workers who have behavioural support expertise.
The BSO Behaviour Support Specialist (BSS) role will work in collaboration with the Behaviour Support Outreach team members to support clients with Behavioural and Psychological Symptoms of Dementia residing in the community and long-term care sector.
The BSS clinician will be part of the resource team and be expected to cover all of the Ontario Health Teams in the Toronto sub regions as needed.
The BSO Behaviour Support Specialist will work to support the BSO team and its Clinician Leaders to complete client observations and assessments, recommend appropriate interventions, model strategies and assist with transitions across sectors.
This role will support existing BSO clinicians and help address the management of systemic pressures (i.e. long waitlists, frequent trips to the ER, etc.). This position will help address two major functions that align with the MOHLTC BSO Strategy.These include:
- Integrated Service Delivery
- Capacity Building
Responsibilities include but are not limited to:
- Participates in relevant data collection, education and research activities, and performs duties consistent with the job classification, as required.
- Provides comprehensive clinical assessments with a focus on identifying factors contributing to responsive behaviors utilizing a biopsychosocial
- Collaborates with client, care partners, and the client's circle of care to develop and disseminate individualized behavior management plans in the community and longterm care sectors.
- Ensures the protection of client rights and acting in an advocacy role for the resident/clients.
Qualifications include but are not limited to:
- Completion of a Master's degree program in a healthcare profession.
- Current registration/licensure in good standing as required by a professional body.
- Minimum three (3) years related experience in geriatrics, communitybased seniors care and behavioural management
- Experience/familiarity with Resident Assessment Instrument—Home Care (RAI-HC) and behavioural, cognitive, psychosocial, and environmental assessments relating to responsive behaviours
- Demonstrated expertise and knowledge of working with individuals with various types of dementia, delirium, and those with acute behavioural change
- Demonstrated competence in the use of standard psychogeriatric assessment tools and behavioural support management approaches
- Demonstrated excellent clinicalreasoning, critical thinking and problemsolving skills
- Training in or knowledge of Gentle Persuasive Approach (GPA), P.I.E.C.E.S. U-First, Montessori Approach
- Effective assessment, intervention, client centered and collaborative care planning skills
- Experience providing caregiver education, support and counselling relating to caring for a person with responsive behaviours
- Demonstrated experience working collaboratively with primary care and specialists in the care of older adults with responsive behaviours
- Ability and willingness to travel in the Toronto central region
- Proficient computer skills (Microsoft Office Suite) and the use electronic communication devices and software pertaining to risk management and RAI documentation
