Role and Facility Information:


Vancouver Coastal Health is recruiting a family physician/small group of family physicians to be the designated provider(s) of primary care services to 10 Acquired Brain Injury (ABI) Clients in a Transitional Housing Program starting in summer of 2023.

The position is 0.5 FTE/5 sessions per week and will involve consultations within the program. There will be access to space for client consultation, clinical assessment and documentation/dictation. This part-time position may be combined with other positions supporting clinical work in Complex Care Housing settings. The anticipated start date is March 2023, for team-based training but is negotiable.

This Acquired Brain Injury Transitional Housing Program is dedicated to transitioning ABI clients from Acute care into a holistic, person-centred transitional housing program for a 3-9 month length of stay.

The physician(s) who accepts this position will play a critical role in supporting the ongoing primary care needs of these clients and in the fostering of an inter-professional shared care team of VCH staff and contracted provider staff (Connect Partners).


Qualifications:


Applicants require eligibility for licensure, in good standing, with the College of Physicians and Surgeons of British Columbia as well as current Canadian Medical Protection Association, or equivalent, coverage.

Physician applicants should also have a Canadian College of Family Physicians (CCFP) designation (or eligibility).

  • Providing team based care along with an interdisciplinary team (MD/RN/SW/allied health).
  • Providing traumainformed care to clients with multiple complex health care needs (i.e. ABI, mental health/substance use) and psychosocial complexity.
  • Providing substance use services, assessments, supports, and opiate agonist treatment.
  • Working a predictable weekday schedule with plan for oncall support, which includes evening, weekend and statutory holiday coverage.
  • Participation in care conference/care planning with service provider team, minimum every 3 month review of care plan
  • Having a knowledge of community services and specialists to support this population an asset.

Demonstrates knowledge and skills in the following areas, or a willingness to develop such skills and knowledge:

  • Health promotion and prevention strategies.
  • Quadriplegia and neuromuscular disorder
  • Tracheostomy and ventilator care
  • Communicable diseases, including HIV, Hepatitis C
  • A desire to serve complex clients.
  • An interest in supporting chronic disease management models of service delivery.
  • Trauma informed practice and culturally safe care for Indigenous peoples
  • Awareness of and sensitivity to patients from diverse ethnic backgrounds and/or with diverse cultural beliefs and practices.
  • LGBTQ2S healthcare
  • Comprehensive family practice background; and
  • Current Opiate Agonist Treatment prescriber exemption is ideal or willingness to obtain an exemption.

Remuneration:
Remuneration will be in accordance with the Doctors of BC/MSP negotiated sessional rates
