The Clinical Manager operates as a member of Stedman Community Hospice Leadership Team.

Guided by SJLCB's Mission, Vision and Values, the Clinical Manager will be accountable to the Director, Stedman Community Hospice and Palliative Care Outreach and is responsible for maintaining clinical standards of practice in accordance with each staff member's designation.

The Manager will ensure safe, consistent, high-quality, competent and compassionate care is provided to all patients of Stedman Community Hospice by overseeing day-to-day clinical operations and ensuring compliance with the College of Nurses of Ontario's Standards, Infection Control standards, Ontario labour laws, and health and safety legislation.


Key Accountabilities and Success Criteria
Reporting to the Director, Stedman Community Hospice & Palliative Care Outreach and working closely with the Hospice Leadership team, this role is responsible for:

Clinical Care

  • Oversees direct patient care provision and patient status changes
  • Oversees admissions to hospice through regular communications with community partners and the Stedman Hospice Palliative Care Outreach team
  • Completes patient assessments in the community as needed to facilitate inpatient hospice admission
  • Oversees and manages the inpatient hospice waitlist by liaising with the Clinical Navigator
  • Liaises with Home and Community Care Coordinators to ensure patient needs are met
  • Ensures that the inpatient hospice is staffed according to the minimum requirements but also according to patient care needs
  • Attends clinical rounds and facilitates daily staff huddle
  • As appropriate, participates in patient and family meetings to optimize physical, psychological, social, and spiritual wellness of patients and their families
  • Acting as the Pharmacy lead, monitors the hospice medication administration system in accordance with Health Canada's Controlled Substances Act
  • Monitors health records to ensure compliance with College of Nurses of Ontario clinical standards of practice
  • Participates in the development, review, and revision of clinical policies and procedures
  • Meets with interested and potential residents and families as needed to discuss inpatient hospice care; liaise and refer to appropriate supports as needed

Leadership

  • Liaise with and report to the Director, Stedman Community Hospice and Palliative Care Outreach
  • Collaborates with the Clinical Practice and Learning Specialist to review and update IPAC policies and procedures and manage prevention of staff and patient spread; a campuswide approach will be taken as appropriate
  • Complete performance appraisals of clinical staff
  • Support the Clinical Practice and Learning Specialist in the education, orientation, training and education plan development of clinical staff
  • Provides support to the Hospice Director in maintaining a quality assurance program to optimize the standards of care
  • Provide opportunities for team building to create a positive work environment
  • Organize clinical staff meetings as appropriate
  • Uphold the Code of Ethics and the Regulated Health Professions Act of Ontario
  • Assume oncall duty after regular working hours and weekends and statutory holidays on a rotating basis

Administrative

  • Support tracking of statistics and results reporting
  • Oversees inventory control and purchase of all clinical supplies
  • Ensures staff have fire safety knowledge and training and participate in regular fire drills
  • Ensures, in collaboration with the campus of care Facilities department, that monthly fire drills are completed and signed off
  • Participate in professional growth and development opportunities
