Job Summary:
Support individuals in various aspects of their lives to enhance independence and overall quality of life. This position is open to candidates of all genders and requires union membership.
Qualifications:
Education, Training & Experience
  • Certificate in related human/social service field
  • One (1) year of recent related experience
  • Or an equivalent combination of education, training, and experience
  • Level 1 First Aid Certificate
Key duties and responsibilities:
  • Participate in assessment, goal setting, and program planning for the individual
  • Document, implement, and suggest changes to program evaluations
  • Provide tailored life skills training focusing on emotional regulation, meal planning, personal care, and budgeting
  • Support person-centered activities aligned with individual goals and preferences
  • Pro-actively handle potential emergency situations using crisis intervention strategies
  • Offer emotional support and assistance with daily living activities
  • Administer medications and assist with medical appointments
  • Perform housekeeping duties and maintain documentation
  • Identify community services that align with individual preferences
  • Liaise with other agencies, professionals, and the community as directed
  • Perform other related duties as assigned
