- Using standards of promoting health, building individual/community capacity, building relationships, facilitating access and equity, and professional responsibility and accountability, the nursing duties will include the following:
- Provides counseling and consultation to individuals, families and groups on sexuality, birth control, pregnancy options, relationships, sexual orientation, needle exchange/harm reduction related issues, and testing and treatment for sexually transmitted diseases, including HIV/AIDS.
- Under medical directives, provide assessment, testing, treatment and immunizations for sexually transmitted infections.
- Conducts Point of Care HIV testing.
- Provides mobile harm reduction and sexual health services for clients.
- Assesses, plans and engages in health promotion initiatives and service delivery with identified priority populations.
- Liaises and collaborates with community partners such as withdrawal management centers, youth focused agencies and programs, and other applicable health and social service providers to develop and implement a comprehensive harm reduction strategy across Halton Region.
- Investigates and maintains surveillance of reports of infectious disease (STIs), including ongoing case management and contact tracing.
- Functions as a member of a multidisciplinary team and is expected to keep accurate health records and statistical information. Provide input as requested to inform the surveillance reports and relevant Ministry reports.
- Performs other duties as assigned.
- Bachelor of Science in Nursing.
- Hold a current Certificate of Annual Registration from the College of Nurses of Ontario.
- Ability to communicate effectively with internal and external groups to accomplish client specific and organizational goals and objectives.
- Knowledge and experience in use of social justice, equity and inclusion concepts, Social Determinants of Health (SDoH), health promotion, community development capacity building, and service coordination.
- Experience with electronic documentation systems (Siebel, TELUS CHR, Panorama, iPHIS, Electronic Medical Records).
- Two (2) years of relevant experience in public health setting.
- Comfort and experience engaging with high-risk youth, individuals who use injection and inhalation drugs, and other vulnerable populations.
- Recent clinical nursing experience including specimen collection and phlebotomy.
- Counseling experience in the areas of birth control, pregnancy, sexual orientation, and sexually transmitted infections (STIs) including HIV/AIDS, and STI case management.
- Project Management experience and experience utilizing Evidence Informed Decision Making.
- Flexibility to work outside of core business hours including evenings, weekends and holidays is required.
- Travel will be required, incumbent must provide their own transportation.
- The incumbent is encouraged to provide a current complete record of immunization according to the Ontario provincial immunization schedule.
- The successful candidate will be made an offer of employment on condition that the Regional Municipality of Halton receives a current (obtained within the past six (6) months), original and acceptable Criminal Records Check, including Vulnerable Sector Screening by the first day of employment.
