Job Summary


Within the context of a client and family centered care model and in accordance with established vision and values of the organization, the Community Care Assistant works under the direction of the health care team to provide services to clients.

Assists clients with varied levels of care, observes, and reports significant changes in the client's presentation.

Collaborates with the health care team, clients and their caregivers to maximize independence with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).

Motivates, encourages, and involves clients in their care. Provides input and feedback in the care planning process to the health care team.

The Sunshine Coast is a 180 km stretch of paradise that's tucked into the southwest corner of mainland British Columbia and a 40 minute ferry crossing from Vancouver.

EXAMPLES OF DUTIES & RESPONSIBILITIES

  • Provides appropriate levels of assistance to clients with their activities of daily living such as personal care tasks (bathing, dressing, grooming, shaving, hair and skin care, meal preparation and nutritional support). Assistance with personal equipment and appliances, transferring, and toileting. Ensures activities are carried out correctly and in a safe manner, and documents the care that has been provided to promote maximum client independence.
  • Supports and assists with IADL tasks as per client specific care plan and established policies and procedures.
  • Performs assignable tasks as per the client's care plan. Supports and promotes maximum self-care and independence in collaboration with the health care team. Recognizes clients' unique needs and abilities to attain goals as established with the health care team, clients and caregivers.
  • Performs delegated tasks as trained; such as catheter, bowel care, medication administration, and tube feeding, maintaining exercise and mobilization routines.
  • Observes clients' presentation and environment, living arrangements, caregiver supports, and reports observations to Nurse and/or to the health care team.
  • Provides input and feedback by participating in team care conferences, supports and implements changes to the care plan while collaborating with the healthcare team.
  • Ensures client safety by adhering to infection control, occupational health and safety standards and reporting safety events.
  • May assist with falls prevention as per the care plan.
  • Attends and participates in staff development programs, team meetings, and other meetings as required.
  • Carries out responsibilities in accordance with health and safety requirements.
  • Performs other related duties as assigned.

QUALIFICATIONS

Education & Experience
Graduation from a recognized Health Care Assistant Program. Registered with BC Community Health Worker and Care Aide Registry. Valid BC Drivers License. Local travel requires the use of a personal vehicle. Experience interacting with persons with dementia, geriatric and/or palliative population will be an asset.


Knowledge, Skills & Abilities
Basic knowledge of client-centered care and promoting independence

Knowledge and ability to operate mechanical lifts, following safe work procedures

Basic computer literacy to operate a computerized client care information system.

Basic knowledge of self-care and motivational support

Demonstrates correct use of body-mechanics and knowledge of injury prevention.

Communicates effectively and in a culturally safe manner with client, caregivers and health care team.
