    We are seeking a highly skilled Community Care Coordinator to join our team. As a Community Care Coordinator, you will play a vital role in supporting clients who are being discharged from hospitals and connecting them with the necessary resources to thrive in their communities.



    About the Role


    • Maintain a client caseload and develop personalized care plans that address each individual's unique needs and goals.

    • Collaborate with internal and external stakeholders to ensure seamless service delivery and achieve optimal outcomes for clients.

    • Develop strong relationships with clients, families, and community partners to build trust and foster a sense of belonging.

    The Ideal Candidate


    • Diploma in health service field or equivalent qualification.

    • Member in good standing with one of the five Colleges regulated to perform in Ontario (e.g., College of Social Workers).

    • A minimum of one year experience working in mental health environment preferred but not necessary if have direct personal experience living with mental illness etc..
