Position Description:
SUMMARY:
As an integrated member of the SJHH Halton - Seniors Mental Health Outreach (SMHO) team, this role will work with health and social support providers and a wide range of organizations who serve older persons with complex mental health, dementia, addiction and physical needs and associated responsive behaviours.
This role serves and supports providers in a variety of settings including Long Term Care Homes, adult day programs, supports for daily living residences, community support agencies working in client/patient homes and Home and Community Care etc.
QUALIFICATIONS:
- Master's preferred. Minimum university degree as a Regulated Health Professional and/or relevent health science degree (e.g. gerontology)
- Minimum of 5 years experience in specialized geriatric psychiatry and/or medicine to include knowledge of the multihealth sector issues facing the target
- Demonstrated experience and advanced skills in geriatric psychiatry/medicine screening, assessment, care planning and the development of holistic intervention and management strategies
- Advanced facilitation skills, coaching, written and oral communication and conflict resolution skills
- Innovative individual with enthusiasm, flexibility and demonstrated skills and commitment to effective team work and continuous learning and quality improvement
- Working knowledge of computer programs including MS Word and PowerPoint
- Valid driver's license and fulltime use of own car is required
KEY FUNCTIONS:
- Develop, provide and facilitate responsive training and learning opportunities for staff in Haltonbased Long Term Care Homes and MOHLTC funded community support agencies and Home and Community Care.
- Work collaboratively to facilitate the development and growth of networks, communities of practice, partnerships and collaborative practices that enhance the service and resources for older adults with cognitive/mental health and addiction needs and for their service providers
- Help to identify gaps and integrate cross sector services and solutions to better meet older adult/service provider needs
- Effectively support and coach staff to understand behaviour and psychological symptoms of dementia and mental illness in older adults and to develop care and support strategies
