Role Description:

Reporting to the Population Health Director and working closely with all members of the

Population Health Service, the Program Manager will support the Population Health Service in program development, delivery, and reporting for their Health Assessment, Health Promotion, and Health Protection arms.


Roles and Responsibilities:


  • Manage and develop the workflow of the population health service in collaboration, with the Director of Population Health Service.
  • Support the Population Health Service with data management and enhancing infrastructure.
  • Manage and oversee administrative personnel within the Population Health Service.
  • Support the Director of Population Health Services in the development and implementation of a Population Health strategy, policies and procedures.
  • Manage a portfolio of grants, including monitoring and measuring impact, analyzing reports to ensure appropriate financial management and deliverables, and reviewing grant payments.
  • Build and maintain systems for accurate tracking for auditing and reporting purposes, including financial reporting, in collaboration with ICHA's finance and accounting personnel.
  • Develop guidance documents for shelter partners with Population Health physicians, nurses, and health promotion personnel.
  • Build resource infrastructure by developing manuals, scripts and process maps.
  • Oversee and onboard volunteers and learners within the population health portfolio.
  • Support development of new and existing internal/external providers in line with program.
  • Review and issue resolution and problemsolving with all personnel.
  • Contribute to the development, evaluation and implementation of new processes and systems to improve program management.
  • Facilitate education webinars for partners and staff.

Qualifications:


  • 35 years in business administration, project management, etc.
  • Knowledge of applicable acts, legislation and policies; project management and change management methodologies; business readiness and risk management principles; etc.
  • Experience with issues management, proactive problem solving and highlevel communication with multiple stakeholders.
  • Strong grant writing skills and budget development.
  • Selfmotivated, problem solver and analytical thinker.
  • Excellent verbal and written communication skills.
  • Innate curiosity and ability to selfstart in an ambiguous and rapidlychanging environment.

Salary:
$80,000.00-$90,000.00 per year


Schedule:

  • 8 hour shift
  • Monday to Friday
