ONA-FULL-TIME REGISTERED PRACTICAL NURSE- MDS-RAI COORDINATOR
BLACKADAR CONTINUING CARE CENTRE
Reporting to the Director of Care, the RPN will enhance the effective management and quality of nursing practice in keeping with the Standards of the College of Nurses of Ontario, Long Term Care Standards and Extendicare Standards.
Responsibilities
- Coordinates the completion of MDS assessment on all Residents within established timelines.
- Understands MDS assessment integration with existing clinical tools and provides leadership in streamlining processes.
- Understands and provides clinical analysis of data at different levels of aggression. (Resident, Home, etc.).
- Articulates for staff strength of MDS as clinical assessment tool including RAP triggers and care plans enhancing quality care.
- Articulates the financial implications of coding inconsistencies.
- Acts as key contact/ liaison with: CIHI/PCC or others as appropriate on RAI data.
- Develops Home specific material within the PCC program as required.
- Locks/Unlocks RAI individual assessments as required.
- Batches and submits assessments quarterly to appropriate data repository.
- Responds to all submission errors.
- Facilitates response and correction of submission errors with Home staff.
- Coaches key Home personnel to build capacity for understanding RUG Resident classification levels.
- Develops and introduces processes, and systems to improve accuracy of RAI content.
- Facilitates audits of RAI assessments for content accuracy.
- Develops systems to decrease coding error rates.
- Participates in strategic planning for program development and improvement.
- Keeps current on RAI trends.
- Responsible for Clinical and Admin Setup role to support MDS within PCC software program.
- Interprets reports generated by CIHI and provides clinical analysis for Home.
- Understands the development of RAP's, quality indicators, and RUG case mix products
Qualifications
- Must have MDS-RAI knowledge, experience, skills and abilities.
- Appropriate current certificate of competence from the College of Nurses of Ontario required.
- Registration or eligible for registration with the Registered Practical Nurses Association of Ontario.
- Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices.
- Effective human relation skills and demonstrated commitment to working with seniors.
- Effective communication skills and ability to work independently and collaboratively are essential.
