EMPLOYMENT OPPORTUNITY
The Sunshine Coast Division of Family Practice is offering an exciting opportunity for a Primary Care Network (PCN) Project Manager to support the developing network on the Sunshine Coast.
Divisions of Family Practice were created through collaboration between the Ministry of Health and the BC Medical Association (Doctors of BC) and are community-based groups of family physicians working together to achieve common health care goals.
The Sunshine Coast Division of Family Practice represents primary care physicians across multiple settings in Sechelt, Gibsons, Pender Harbour and neighboring communities on the Sunshine Coast.
Associate members include retired family physicians, family medicine residents, other specialists, nurse practitioners and midwives.
The PCN Project Manager is responsible for leading and supporting the development, implementation and evaluation of the Sunshine Coast PCN in accordance with the strategic direction of the PCN Steering Committee in alignment with Ministry of Health PCN attributes.
- responsible for the overall project management of implementation of operations and evaluative initiatives of the Sunshine Coast PCN.
- provides leadership in program development and evolution of the PCN.
- responsible for management of PCN administrative team members.
- demonstrates and promotes a commitment to cultural safety and humility within the PCN such that services are delivered accordingly.
- actively participates and supports the effective governance of the PCN by supporting leadership committees and working groups.
- develops and maintains strong relationships with physicians, their office teams, and allied care providers, particularly those that are part of the PCN.
- establishes and monitors all necessary operational procedures/resources.
- works in collaboration with PCN change management resources available to ensure effective change management through engagement of all people impacted by implementation of the PCN including health care providers, First Nations and other Indigenous people and communities in the PCN.
- develops & supports establishment of evaluation processes, including working with existing PMH evaluation frameworks.
- supports alignment of the PCN initiative with other services in the community.
- participates in identifying opportunities for improvement to facilitate access and improved quality of care for populations served by the PCN.
- promotes positive communications and collaborative practice to improve efficiencies and works with teams to resolve conflict as required.
- works collaboratively with representatives and partners, the Ministry, and other communities.
- other duties as assigned.
Reporting to the Executive Director, the Division PCN Project Manager will be also be broadly accountable to the PCN Steering Committee for the development, implementation and evaluation of the Sunshine Coast PCN Service Plan.
Preferred qualifications of the successful applicant include:
- Postsecondary degree in a health or leadership related discipline. Master's degree in a relevant health or administrative field preferred. Experience in healthcare and project management is highly desirable as is an understanding of primary care and/or PCN. Other combinations of education and experience may be considered including community development.
- A minimum of five (5) years previous collaborative and distributed leadership experience, ideally in a health care setting or similar.
- Demonstrated experience in integrated health service delivery planning and design, project and program management, implementation and evaluation.
- Demonstrated experience working in complex systems and familiarity with medical models.
- Demonstrated experience with budgeting, reporting, financial management.
- Demonstrated experience in team leadership.
- Demonstrated commitment to positive, collaborative, interdisciplinary teamwork.
