Account Lead, National Disability Services (Remote)

As an Account Lead you will facilitate effective and timely communication between our clinical teams and referral sources as we support clients in their physical and/or mental health rehabilitation.

KEY CONTRIBUTIONS:


  • Ensure the needs of the client are understood, the assessment findings are communicated to the referral source and program goals are clear to everyone
  • Monitor the progress of the client, facilitating communication with the referral source when barriers or challenges are identified and changes to the care plan may be required
  • Liaise with referral sources and/or clinical teams to offer potential solutions to complex client challenges
  • Provide input and feedback to CBI Health leaders to support continuous quality improvement of services
  • Develop strong relationships built on trust with clinical teams and funders/referral sources

WHAT YOU BRING:


  • You thrive in a dynamic, fast paced environment, managing time effectively and efficiently
  • You are proactive, innovative, and resourceful
  • You have excellent listening skills and the ability to influence with strong verbal communication skills
  • You are a collaborative team member and comfortable working independently
  • All postsecondary degrees with health or vocational background are welcome
  • Experience in disability insurance sector, either in insurer or employer role is an asset
  • Postsecondary degree in health and/or experience in vocational rehabilitation, case management and/or care navigation is an asset
  • Ability to communicate in French is also an asset
