    Intensive Case Manager Role Overview

    This position is a dynamic opportunity for an experienced individual to join our team as an Intensive Case Manager. The successful candidate will be responsible for providing comprehensive support services to clients in need, assisting them in achieving stable housing and optimal living conditions.

    The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. They should also have experience working in a case management role and be knowledgeable about community resources and social services.

    Key Responsibilities:

    • The Intensive Case Manager will work closely with assigned families and individuals in a housing program designed to achieve long-term stability. This includes coordinating services such as income maximization, tax preparation, ID acquisition, and community connections.

    • Responds promptly to calls from community agencies according to their caseload requirements.

    • Placing individuals into permanent leased units that meet their specific needs while ensuring seamless integration within the existing system of care.
