Description

Main Activities

There are two homes
- one which provides transition, respite and palliative care. The other which provides a program to at risk moms who are prenatal and postnatal preparing them for transitioning to a new home with their infants.

Provide supportive care to people living with or at high risk for HIV/AIDS who may also be affected by mental illness, addictions, poverty, homeless and other psycho social factors
Provide supportive care through a patient centered model aimed at creating independence and autonomy while fostering an environment of acceptance and understanding
Assist with and support clients with medication adherence
Work with patients on individual goals and care plans aimed at recovery and management of their illness(s)
Provide therapeutic counselling/assistance to clients who may present with multiple issues affecting their ability to cope and manage in the community
Advocate on behalf of clients for additional service and resources, as required
Personal care to patients including, assisting with grooming, bathing, personal hygiene, toileting, wound care, nutrition and all other activities associated with daily living
Establish and maintain current, accurate, confidential files for each client
Educate clients, families and support systems about Sanctum and what can be provided and when to access other resources
Assist with the transition plans from Sanctum into the community set by the client and care team
Follow medical instructions carefully and consistently
Assist with meal preparation and planning, as well as other house hold duties such as cleaning and maintenance
Organize time and resources based on the individual needs of clients
Manage any unanticipated events or unstable situations
