Description

We are looking for:
a full-time, contract Hospice and Bereavement Student (May to August 2023)

Hours of Work: 34 hours/week. M-T 8:30am to 4:30pm and Friday 8:30am to 3:30pm. Hybrid work from home.

Reports to:
Manager, Social Work

Responsibilities:

Onboard new clients and complete initial assessments
Provide supportive counseling
Develop care plans for clients and families
Complete Hospice training
Assist with developing educational materials
Participate in outreach and marketing activities
Escort clients to medical appointments and delivery groceries
Provide training to clients on the use of tablets
Assist with organization and delivery of educational and social programs (LGT)

Qualifications:

Two years' completion of a BSW or SSW program OR one year completion of a MSW program
Some experience with geriatric population and end of life an asset
Russian speaking an asset but not required
